When can you apply for FVIP Provider Certification?
FVIP Provider applications are accepted January 1st and July 1st annually.

When will you be added to the certified provider list?
Once GCFV staff has reviewed your application and all requirements are met. Usually takes 2 weeks from receipt of application.
- All required documents are submitted.
- Pass the background check.
- A certified facilitator has been identified to conduct classes.

Once certified, when do you have to renew certification?
Providers must recertify every 2 years from the date of initial certification. The same requirements must be met for recertification.

Where do I find the initial provider certification application and how do I apply?
Program applications are accepted on January 1 and July 1 annually.
