FVIP Provider Certification
Initial FVIP Program Owner Certification – Step‑by‑Step Process
1. Attend the FVIP Informational Session
- Required first step for all Program Owner applicants.
- Must submit a signed authorization for background checks.
2. Complete the Initial Background Check
- Must be approved before enrolling in FVIP Basics.
- Includes criminal history, Protective Order Registry, and DMV driving history.
3. Complete FVIP Basics (14 Hours)
- Mandatory Commission‑delivered training.
- Must be completed before any other requirements.
4. Identify All Judicial Circuits of Operation
- One application is submitted, but the applicant must list every judicial circuit where the program will operate.
5. Submit Mission Statement
- Must include the program’s purpose and the role of FVIP within the organization.
6. Provide Background, History & Experience
- Must include the program owner’s experience related to domestic violence.
7. Disclose All Business Interests
- Must list all other businesses, ownership ventures, partners, co‑owners, investors, or affiliates with substantial shared business interests.
8. Submit All Program Locations, Class Schedules & Assigned Facilitators
- Must include addresses, proposed schedules, and certified Facilitators.
9. Submit Class Fee Schedule & Indigent Fee Reduction Plan
- Must include written fee schedule and sliding‑scale indigent plan.
- Must be provided to all referral sources.
10. Provide Proof of General Liability Insurance
- Minimum coverage: $1,000,000.
- Government entities may request a waiver.
11. Submit Victim Liaison Contract or Employment Agreement
- Must be with a CJCC‑certified DV program, Commission‑approved community‑based program, or approved DV advocate.
- Must maintain a victim liaison at all times.
12. Sign and Submit Principles of Practice
- Must be displayed at all program locations and visible during virtual classes.
13. Sign and Submit Code of Ethics
- Required at certification and recertification.
14. Meet Notice & Disclosure Requirements
Program Owners must immediately report:
- Arrests, charges, pleas, protective orders
- DFCS reports
- Licensing sanctions
- Civil actions alleging harassment, misconduct, dishonesty, fraud, theft, or DV
15. Participate in Coordinated Community Response (CCR)
Must attend two CCR meetings annually (e.g., Task Force, Family Connections, DV court observation, victim liaison meeting).
16. Submit Application + Required Fees
- $250 nonrefundable application fee for one judicial circuit.
- $50 for each additional judicial circuit.
- $100 for online classes.
17. Await Commission Review
Commission notifies applicant within 30 days whether the application is approved, denied, incomplete, or requires more time.
